Learn the ins and out of creating an Advance Directive with local attorney Cora Rose!
 

In Oregon, the medical Power of Attorney is known as the Advance Directive. The Advance Directive is a document that allows you to make it clear what you would like to happen with your medical care if you ever become so sick that you’re not able to speak for yourself.

Many people think advance directives are only for those with a terminal illness. However, unexpected situations can happen to anyone at any age. If you experience a devastating accident or illness that renders you unconscious or unable to communicate, an advance directive can help you make sure your wishes about your medical care are known, and that the person you want to make decisions for you can.
 
This workshop will run from 10:30am to 2pm with a noon lunch break (doors open at 10am). A lunch option will be provided for all attendees—please make sure to select your preferred meal when you register!

Who this workshop is for:

Oregon residents who are competent adults or emancipated minors who want to plan for the unexpected and help their loved ones care for them
 
What to expect:
  • Learn about the Oregon Advance Directive and Appointment of Healthcare Representative form, process, and your options
  • A chance to ask general legal and practical questions and walk through the form step-by-step
  • An opportunity to reflect before completing and notarizing your directive, if you feel ready.
 
This workshop is NOT for:
People who already have completed a comprehensive estate plan that includes a medical Power of Attorney or Advance Directive
People who seek a POLST (which must be completed with your medical provider)
Those looking to create a will or estate plan (the topic will be limited to last wishes for healthcare decision-making)
